Somebody had to do it for him, because bending your hand like that to make it comfortable to draw the giraffe is quite impossible, if your not double jointed, or he is just amazing and was able to draw a giraffe upside-down.

Since it is her left arm shown on the picture, I tried to put mine in a motion where the fingers are facing towards me. It is possible, but of course would probably look nicer if someone else did it for the person.

You do make a point, I didn't quite think if it was the left hand or not, I automatically thought it was the right hand, because I'm right handed.
