Yes, make sure to downgrade your GTA IV to 1.0.4.0 before using this, and it doesn't appear to work on EFLC :/

Yeah, but then you don't get Multiplayer, and the older patches are buggy and don't run well on all systems :| I'd rather wait for the modders to update the mod...

GTA IV on pc is buggy already, hell even on my system it lags like a bitch and bugs out. Its very hit and miss. I wouldn't even consider attempting to mod the graphics considering how frigid the coding is.
